POSITION SUMMARY
A Home Health Agency Speech Therapist is a professional practitioner that plans and provides Speech- Language therapy services to clients in their homes in accordance with the Plan of Care and under the direction of the client’s physician. The Speech-Language Therapist performs functions that require substantial specialized knowledge, judgment, and skill, must be able to make judgments accordingly.
CLINICAL RESPONSIBILITIES
Provides services and education/training to clients in their homes following the established Plan of Care and the specific functions identified by the Speech-Language Pathologist.
Compliance with OASIS protocol per Medicare and State regulations
Conducts appropriate evaluations, assesses home safety environment and identifies equipment needs relative to speech/language function.
Identifies functional speech defects and establishes a plan of care to improve patient’s function.
Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
Recommends communication devices/aides as indicated.
Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care and contributes to the development of the plan of care.
Participates in patient/family education regarding home health care.
Trains patient in accordance with home health agency policies and procedures.
Accurately completes all necessary documentation on appropriate home care record/forms and submits to Options for Referrals within 72 hours.
As appropriate participates in case conferences, inservices and other meetings required ensuring coordinated and comprehensive care.
ORGANIZATIONAL STANDARDS
Complies with Options for Referrals policies and procedures.
Maintains personal health status requirements in relation to job position
Maintains current personnel file information and provides information to agency in a timely manner.
QUALIFICATIONS
Master degree in the field of Speech Pathology approved by the American Speech-Language-Hearing Association (ASHA).
Master’s level professionals with a strong knowledge base in speech-language/literacy development and speech-language/literacy disorder, as well as a strong skill set in diagnosis, intervention, and workload management
Current and unrestricted license to practice speech-language pathology in the State of CA.
One (1) year experience of speech-language pathology experience preferred.
Current CPR certification.
Must be familiar with general use and functions of the computer such as user names and passwords concepts; internet; e-mail; navigation of computer desktop.
OTHER
Licensed driver with automobile insured in accordance with state requirements.
Required professional liability insurance
Must be able to adapt to the patient’s needs and schedule changes.
