    What is Options For Referrals?

    Established in 2003 as Options for Rehab, coordinating physical, occupational and speech therapy services for patients at home. We have grown into a leading referral service and trusted partner to home health agencies

    Necessary therapeutic modalities and care coordination is delivered in the comfort of the patients home.

    Many of our patients are referred by local home health agencies in the San Bernardino, Riverside, West Los Angeles and North Orange counties. A large percentage of our patients are referred to us because of positive word-of-mouth reputation in our communities.

    How can we refer patients to OFR?

    If you have a contract with us, you can just send patients to us via email or fax, OR call the Intake team at 909-463-7820

    If you do not have a contract with us, we can enter into one within 2 days – contact HR at 909-463-7820

    How soon can you see our patients?

    Patients are called immediately on receiving referrals. We strive to have your patients seen within 24 hours,and definitely with in 48 to 72 hours. By logging on to our site you will be able to access all information about the patient in real time. The therapist that has seen the patient can be contacted directly for a clinical conference.

    How soon do we get receive documentation from OFR?

    A majority of our clinicians document on site on their iPads / Androids. as soon as they upload the documentation you can integrate the documentation in your system, or we can assist in the upload. Since the signature of the patient can be traced by a GPS and can be taken on the clinicians ‘smart phones’ you can have validation of the visit done.

    Will you document on our (HHAgency) e- forms?

    We strive to follow care protocols for each agency. However our Jility e-forms are designed for relevant therapies with their effects on function. A sample of our forms can be viewed on our site.

    Each therapist goes through an extensive training on the system, it is logistically very hard to accommodate a request from a myriad of agencies.

    Who Is Options For Referrals for?

    OFR is for frail patients in need of efficient, medically necessary and restorative rehabilitative care If you are homebound, you are eligible to receive our services through Medicare Part A. We have alliances with home health agencies, that have contracts with HMO’s, PPO’s and insurance companies who can help facilitate the services you need. If you are in need of (self paid) consultations – we offer the services at your convenience too.

    Our licensed therapists have the depth of experience nessessary in helping patients recover from most geriatric related disabilities viz. fall injuries, joint replacement surgeries, and stroke. The therapy services assist the patient in learning how to restore function and live independently with minimum assistance if needed.

    How do I start services with OFR?

    In coordination with the your own local physician, we would need a verbal or written prescription after your in-person visit with him/her.

    A phone call to our Intake coordinators at 909-463-7820 will start the process. We will work on your eligibility, admission and billing with our alliances.

    When can I be seen?

    As soon as the necessary order is received from the physician, we are able to find out about eligibility requirements of the home health agency within 3-4 hrs. You will receive a call from the therapist within 24 hours.

    Am I strong enough for therapy?

    When we do our initial assessment, we will be performing some standardized tests to assess your tolerance and safety with activities.

    What do I do to start?

    We will contact you, arrange for an interview (over the phone, at the office or course at a Starbucks close to your home or workplace). If you fit in our company’s culture, We would then need to verify your license and standing with the state board.

    You could work for us as an independent contractor or employee; After you submit/sign all the required forms of the New Hire Folder, you are ready to start – when ever you want.

    You will need Stethoscope, Sphygmomanometer, Pen light, Pulse-Oximeter, Reflex hammer, Bandage scissors, Goniometer, Retractable tape measure, Skin marker, Gait belt, Surface disinfectant, Barriers to place equipment .

    What are the costs involved in being a home health therapist?

    Professional liability approx. $75-100 annually, car insurance, physical examination approx. 75$, TB test approx. 15-30$ at industrial clinics and of course your professional license renewal.

    What do I have to do after I join OFR?

    Electronic documentation Training via “Go to Meeting” or in person. You will need an Android tablet or iPad if you wish to document while on the field.

    Otherwise your smart phone can capture the patients signature for the visit done.

    We will need a photograph of you identifying you as a OFR representative All the necessary procedures of writing physician orders, communication notes, signage on route sheets and invoicing is automated.

    You will have access to any/all therapists to get a feel of ‘work at OFR’ and the clinical director for “a second opinion” if you so wish.

    Since the volume of patients is not sustainable by therapist visits, you will be matched up 1-2 PTAS, COTAs to assist you with your workload

    What office equipment will I need to get started?

    There is not much that is needed to get started but at least the following is absolutely necessary:

    • texting, emailing equipment
    • cell phone
    • Multifunction laser printer/fax
    • Computer, tablet with internet access
    Do I have to quit my current job?

    One of the best aspects of our practice is that you can continue to work in your current job as you build volume of patients. or choose to take patients close to home or your work site for the extra money (that we all need)

    How did OFR start? Who owns OFR?

    Vazira Kanga PT, MHA (about story behind) is the sole owner of OFR, started OFR in 2000 in Rancho Cucamonga, CA. She continues to be very active in its development, and provides patient care nearly every day.

    Can I contact current Therapists to discuss their experience with OFR?

    We will be happy to provide you with all the contact information you need to discuss with any or all of the currently active therapists. We are proud of how each of them has done and encourage you to chat with them at your convenience.

    I have heard of documentation in Home Health being "painfully long"

    You went to school to be a therapist, not a scribe.

    Will I be seeing patients for different agencies? will i have to document differently for these agencies

    You will be seeing patients for a number of different agencies – but only documenting on Jility.

    How often do i have to come OFR?

    No more than 1-2 times a year. Our therapists telecommute, attend webinars, and are updated about events on intra office E-mails.
